By signing this form, I acknowledge and agree that participation in the Empower Her Ministries® Scholarship Pageant does not guarantee placement, awards, scholarships, or titles of any kind. Selection of winners is at the sole discretion of the judges, and all decisions are final and not subject to appeal. I further agree not to disclose, reproduce, or share confidential materials, score sheets, or internal pageant processes.
Assumption of Risk & Liability Waiver
I understand and voluntarily assume all risks related to my participation in this event, including but not limited to travel, rehearsals, performances, and associated activities. I release and hold harmless Empower Her Ministries®, its directors, volunteers, staff, sponsors, and venue partners from any and all liability, claims, or damages arising from injury, illness, loss, or other incidents connected to my participation.
Medical Release
In the event of a medical emergency, I authorize Empower Her Ministries® representatives to secure medical treatment for myself/my child. I agree to assume responsibility for all costs incurred.
Code of Conduct Agreement
I agree to abide by the rules of the pageant, conduct myself with respect, and honor the values of Empower Her Ministries®. Failure to comply may result in disqualification without refund.
To confirm your spot in the Empower Her Ministries® Scholarship Pageant Program, a $150 Contestant Commitment Contribution is required. There is a $100 non-refundable fee, which helps cover the cost of mentorship, workshops, and event preparation.
By submitting this form, you acknowledge that:
• The $150 contribution is a commitment to participate in the full program experience (faith encouragement sessions, workshops, rehearsals, and pageant night).
• This contribution is non-returnable under any circumstances, even if a contestant withdraws or is disqualified.
• No awards, scholarships, or titles are guaranteed by participation; all outcomes are determined at the sole discretion of the judges and Empower Her Ministries®
